Transcripts & Diplomas

The District School Board of Niagara (DSBN) Records Centre helps former students request official copies of their transcripts and diplomas. This includes students who attended a DSBN secondary school or Adult Learning Centre, as well as schools from the former Lincoln County (LCBE) and Niagara South (NSBE) school boards.

Before You Get Started

Please read the following important information before making your request:

  • The DSBN Records Centre can only process requests made by former students who last attended a DSBN secondary school or Adult Learning Centre (ALC), including the former LCBE and NSBE
  • If you last attended a secondary school or adult learning centre that was not part of DSBN, please contact that school or school board directly
    • Not sure if your school was part of DSBN? You can check this list – DSBN Secondary Schools – which includes both open and closed schools. 

Important Things to Know:

  • Student transcripts are confidential (private). They can only be released with the student’s written request or consent.
  • To request a transcript, the former student must fill out a request form and provide proof of ID.
  • If the student is 18 or older, a parent or guardian cannot request or receive records without the student’s written permission and ID.
  • If there is a mistake in your transcript (courses or marks), you will need to contact your former school. The Records Centre cannot make changes.
  • The Records Centre is not authorized to print Report Cards. Please contact your former school about these documents.
  • Under privacy laws (MFIPPA), we cannot share student information over the phone.

Records that provide Proof of Secondary School Achievement/Graduation:

Ontario Secondary School Transcript

A Secondary School Transcript lists all courses taken and marks achieved from grades 9 to 12/12B/OAC/13 by a student and will also state the student’s graduation status. If the student is a graduate, the transcript will list the type of diploma awarded and the date that it was granted.

Diplomas

Diplomas are used for providing proof of graduation from Secondary (High) School.

Diplomas provide proof of graduation only and do not contain any course or mark information.

Certificates are used to show that a student attended secondary school but did not complete the requirements for a diploma.

Before You Submit

Include your proof of ID with your DSBN Transcript & Diploma Request form.

Forms received without proof of ID will not be processed until the proof of ID is submitted.

The scan/photo or photocopy of your proof of ID must be easy to read.

Mailing Multiple Addresses?

Enter “See attached list” and provide the mailing addresses in the email that the form and your proof of ID are attached to.

Ordering copies for College or University?

  • Include the full mailing address of the institution with the name of the specific department receiving the transcript (e.g., Admissions)
  • Include any reference/student/application number for the college or university (if you have one)
  • If the college or university has the option to receive scanned transcripts as place holders under the hard copy arrives int he post, include the admissions email address

Submitting the Transcript & Diploma Request Form

Send Request Form and Proof of ID to: transcripts@dsbn.org.

  • Proof of ID can be sent as an attachment by scanning or taking a digital photograph. PDF or JPG files are preferred.
  • Once your records have been located you will be sent instructions via email on how to remit payment using DSBN School Cash Online.
By Mail, Courier or In-Person

Helpful Tips
  • If filling out the form by hand, ensure your handwriting is easy to read to avoid delays or errors
  • The photocopy of your proof of ID must be clear and easy to read
    • Only the front of your ID is required if it includes both your legal name and date of birth
  • Payment must be included as a money order:
    • Make the money order payable to the “District School Board of Niagara”
    • If possible, include your name in the “Memo” section
Fill out a blank form in person at the DSBN Transcript Services Office

Come to the DSBN Education Centre at 191 Carlton Street in St. Catharines during business hours.

September to June (school year): Monday to Friday, 8:30 a.m. to 12:00 p.m and 1:00 p.m. to 4:00 p.m (except statutory holidays)

July and August (summer break): Monday to Thursday 8:30 a.m. to 12:00 p.m. and 1:00 p.m. to 4:00 p.m. (closed Fridays, statutory holidays and during annual summer shutdown, last two weeks of July)

  • Sign in at Reception and ask for directions to the Transcript Services Office.
  • Take a blank Transcript & Diploma Request Form from the wall hanger located on the left of the Transcript Services Office door.
  • Fill out the form by hand and ensure your handwriting is easy to read.
  • Show the Transcript Services Secretary your proof of ID.
    • ID must have both your legal name and your date of birth.
  • For official transcripts and diplomas, be ready to provide either:
    • A money order for the amount owed and made payable to the “District School Board of Niagara”
    • An email address that the payment instructions can be sent to for paying via credit card when the document(s) are ready.
